Read this aloud to the patient before you begin:

"I am going to ask you nine questions about how you have been feeling over the last two weeks, including today. For each one I will read you a short description of an experience or feeling, and I would like you to tell me how often you have felt that way.

There are four possible answers: Not at all, Several days, More than half the days, or Nearly every day. I will show you the options as we go, so there is no need to remember them. There are no right or wrong answers — I am just trying to understand what your experience has been like recently. Do you have any questions before we start?"

How to administer — guidance for clinicians

Read each question aloud exactly as written. Do not paraphrase or summarise — the validated wording must be preserved to ensure scoring accuracy.

Point to or gesture toward the scale strip above while asking each question, so the patient can see the four response options throughout.

If the patient is unsure, remind them: "Just your best estimate is fine — think about the past two weeks as a whole." Do not lead or suggest an answer.

If the patient gives a verbal response ("sometimes", "a lot"), map it to the closest option and confirm: "So that would be 'Several days' — is that right?"

Q9 requires a follow-up. If the patient scores 1, 2, or 3 on Question 9, ask immediately: "Can you tell me a bit more about those thoughts?" Do not move on without exploring this response.
!Administer in a private setting. Ensure you have time after completion to discuss the scores, particularly if the patient scores in the moderate-to-severe range or gives any response above 0 on Q9.
